  I would like to join the Altar Server Ministry and serve our parish community by assisting the priest during the liturgy.
  
  I am a Catholic youth in the third grade or older who has received Eucharist, and I agree to:
  
  Be attentive during the liturgy, answering prayers and responses.
  
  Be appropriately dressed and groomed.
  
  I will be committed to my assigned schedule. If I am unable to make it, I will find a replacement and inform the Ministry coordinator.
  
  Be present 20 minutes before the liturgy begins and introduce myself to the priest (who will tell me if there is anything speical I should know).
  
  Sign-in on the Altar Server List each time I serve
  
  Attend required trainings.
  
  Inform the Altar Server Coordinator of any changes in address, phone or email.
  
  Have parent or guardian permission to for my image as part of the liturgy to be used in parish media (e.g. bulletin or live-stream).
